Add namespace to AssessmentFamily and Assessment

Namespace is being added to AssessmentFamily for the purposes of authorization in the ODS API. A default namespace is assigned to each vendor so they can create and manage vendor specific descriptors. The same approach is being used on assessment family to tie assessments to vendors as well as the student assessment records.
