Increase field length to 32 characters for DisciplineAction.DisciplineActionIdentifier

Boston is having problems guaranteeing unique values for DisciplineAction.DisciplineActionIdentifier and we ask that the field length be increased to nvarchar(32). It is nvarchar(20) now. 

Reason
Our SIS (Follett Aspen) relies on 14 character case-sensitive record identifiers that are also used as identifiers in some Ed-Fi entities.  To guarantee uniqueness in the ODS, which is case-insensitive, the SIS derives 28-character strings from the 14-character identifiers. This works well in all cases except the DisciplineActionIdentifier.

In addition to DisciplineAction, this change request affects the following related tables:

• DisciplineActionDiscipline
• DisciplineActionStaff
• DisciplineActionStudentDisciplineIncidentAssociation

We are implementing a compromise solution that will work for a while, but we really need the field length increased as soon as possible.
