Suggested 'ItemNumber' column in StudentAssessmentItem

In working with Pearson, Delaware has determined while individual questions ('What is the square root of 4') are defined in the metadata (AssessmentItem), those questions may be presented to the student in a different order for each student, and that this order is information used for analysis of the results.  We request that this ItemNumber be reported back to DOE and have extended StudentAssessment to contain the information. 

Sayee was involved in the discussions with Pearson and Delaware Assessment group and asked me to submit this as for possible consideration in core for future.
