Recommend patterns for assessment identifier values

Some assessment providers are creating AssessmentIdentifiers of unnecessary complexity.  For example, we have seen providers adding the assessment provider name into the key, and other possibly unnecessary information.

Some elements could be necessary to generating unique values within the namespace, but adding the provider name is certainly not (the namespace should handle that).

The task would be to provide clearer guidance as to key formation.
