Expand max width of all "Identifier" attributes to be at least 36 characters to accommodate formatted GUIDs

In Ed-Fi 3.0, the "partial key surrogate" pattern has result in the addition of multiple "Identifier" attributes into the primary keys to allow the value to be supplied by external systems.

Currently these attributes are defined in various ways, some limited to as few as 20 characters. These identifiers need to be large enough to accommodate the use of a UUID formatted with hyphens (36 characters).
